Our Services

Adult Day Services

L.I.N.K.S. - (Living Skills, Integration, Networking, Kinship, Self-Advocacy)

L.I.N.K.S. offers individuals with intellectual disabilities, brain injury or chronic mental illness an environment where they can receive assistance with advocacy, integration, improvement in self-help, socialization and adaptive skills. Our unique environment offers client-driven structured activity centered on personal choice, individualized care and goal setting.

Individual Service Plan
A service coordinator will act as your advocate while at Crossroads. An individual service plan will be developed with you, your family/guardian and the support team, based on your desires, needs and goals. This plan will be revised regularly to assess your outcomes. The staff at Crossroads will make referrals as needed.

Admission Criteria

  • You want to receive the services for which you applied
  • Benefiting from the services for which you applied
  • Funding by outside sources or private pay
  • Currently not diagnosed as an alcoholic or drug abuser that prevents you from benefiting from the services you receive 
  • Not harmful to others around you
  • Not listed on the Sex Offender Registry or have a felony on your record
  • 16+ years old if you are not a student
  • 14+ years old if you are a student, and deemed ready for the Day Habilitation program by the School Interdisciplinary Team

Employment Services

Supported Employment helps clients reach their vocational goal and contribute to the greater community. The first step is assisting individuals as they identify career interests and skills that can be applied in local businesses.

Residential Living

Residential Living services are designed to support and provide skills and activities for daily living to adults and children with disabilities and mental illness.
