We sincerely thank you for your support of Crossroads of Western Iowa, and we want you to know that your donations of time, money and support help us to continue the important work we to do to promote empowerment.

L.I.N.K.S. (Living Skills, Integration, Networking, Kinship, Self-Advocacy) offers individuals with intellectual disabilities, brain injury or chronic mental illness an environment where they can receive assistance with advocacy, integration, improvement in self-help, socialization and adaptive skills. Our unique environment offers client-driven structured activity centered on personal choice, individualized care and goal setting.

Individual Service Plan
An individual service plan will be developed with you, your family/guardian and the support team, based on your desires, needs and goals. This plan will be revised regularly to assess your outcomes. The staff at Crossroads will make referrals as needed. A service coordinator will act as your advocate while at Crossroads.
