Covid-19 Vaccine Information for CWI Employees

Hello everyone,

As previously mentioned, Crossroads is developing a plan for Covid-19 vaccination among our staff. Receiving the vaccine is not mandatory, but it will be made available to you all per county guidelines and we encourage you all to consider receiving it.

As the Chief Human Resources Officer, I will work closely with Market Directors to coordinate vaccination plans for individual markets, in partnership with county officials. The vaccine may be available more quickly for employees in some counties than for those in others, so please do be patient with us throughout distribution.

When vaccine doses are available in your county, your Market Director will contact you. On the following page, I’ve outlined your next steps for receiving or declining the vaccine.

There are currently two vaccines available with more in development. Which vaccine you receive will depend on the availability in your area and Crossroads is not favoring one over others. Both the Pfizer and Moderna vaccines are over 94% effective, according to clinical trials, and appear to prevent the spread of the virus. As we work with vulnerable populations, preventing the spread of the virus is a top priority for Crossroads.

Guidelines for Receiving or Declining the Vaccine:

To receive the vaccine, you will need to:
1) Register to receive the vaccine and schedule your first appointment. Registration will be made available to you through your county’s Public Health. Please allow 30 minutes to 1 hour for your appointment.

2) Attend your scheduled appointment to receive the first dose of the vaccine.
- Bring your printed Covid-19 Vaccine Consent Form, Crossroads ID badge, a mask.
- Ensure the administrator completes your Covid-19 Vaccine Consent Form; keep the form.
- Before leaving, schedule the appointment for your second dose. Again, allow 30 minutes to 1 hour for the appointment.

3) Continue to follow Covid-19 safety guidelines, including social distancing and mask-wearing.

4) Attend your second scheduled appointment to receive the second dose of the vaccine.
- Bring your Covid-19 Vaccine Consent Form, Crossroads ID badge, and a mask.
- Ensure the administrator completes your Covid-19 Vaccine Consent Form; keep the form.

5) Continue to follow Covid-19 safety guidelines; this includes social distancing and mask-wearing until the vaccine reaches its full efficacy.

6) Submit your completed Covid-19 Vaccine Consent Form to brittney.smith@cwiowa.org.

To decline the vaccine, you will need to:
1) Complete the Covid-19 Vaccine Declination Form.
2) Submit your completed Covid-19 Vaccine Declination Form to brittney.smith@cwiowa.org.
